Top Five Questions About Fillers

Nowadays, it is no longer a taboo when one goes for filler treatments to enhance their looks. Fillers are becoming a go-to beauty fix with quick and instant results.

Most importantly, fillers are great for maintaining youthfulness. It helps to smoothen wrinkles, fill sunken cheeks, and shape facial contours like making your nose and chin sharper. All of these sound like a perfect beauty remedy, but as with all aesthetic treatments, it is best to have all your doubts answered by a doctor before proceeding with the treatment. 

Today, we asked Dr. Terence Tan, Director of Halley Medical Aesthetics to answer some of the top five questions that our readers have on fillers. 

 

1. What happens if there is an overfilling of my fillers? Can the doctor fix it?

Some dermal fillers like the hyaluronic acid fillers, tend to absorb water within one week of treatment, causing some concern in patients who may not be aware that this may happen. The swelling is generally more obvious in the morning and can be improved by gentle pressure or a mild cold compress.

For cases in which there is an overfill, hyaluronic acid fillers such as Juvederm, Belotero, and Restylane can be dissolved by a doctor using hyaluronidase, which is a naturally occurring enzyme found within our skin. When hyaluronidase is injected into the affected areas, it breaks down the hyaluronic acid rapidly, helping to correct the overfilled areas. If you are concerned about your result, I would recommend going back to the doctor who did the treatment for a thorough assessment.

Hyaluronidase, however, only works for dissolving hyaluronic acid fillers. Speak to your doctor about your concerns so that he can prescribe the most appropriate filler for the area that requires filling. It is also important to fill the area carefully to prevent overfilling.

 

2. Will I be left with worse volume loss after hyaluronic acid fillers are reabsorbed?

There will be no worsening of volume loss after resorption of the hyaluronic acid fillers. Natural hyaluronic acid is constantly produced and broken down in the skin. Studies have shown that modern hyaluronic acid fillers have good tissue integration with the skin with little ill effects.

Volume loss is due to age-related, localised reduction of facial fat and bone loss, aggravated by loosening of the skin. Fillers will not affect the natural ageing process, so there is no need to worry about this.  

 

3. How likely will lumps develop after injecting fillers to the face?

Visible lumpiness after under-eye hyaluronic acid fillers injection, while uncommon, can occur if the skin under the eye is very thin, or if the fillers chosen is too firm, or if too much fillers are used. As such, it is better to choose softer versions of hyaluronic acid fillers when patients have thin under-eye skin. 

Mild lumpiness can also occur if there is bruising after the procedure. This should resolve naturally within a week. It is important not to take certain painkillers like aspirin, and not to consume alcohol immediately before and after treatment as they increase the chance of bruising. Fillers can be used with certain techniques like using a blunt cannula to reduce the risk of lumpiness and bruising.

In general, a good technique coupled with a sensible selection of the fillers type and using an appropriate amount of fillers should reduce the risk of lumpiness after an under-eye filler treatment to a minimum.

 

4. Can I combine fillers with other treatments within the same week?

Yes, you can. For example, Ultherapy and dermal fillers are a great combination for facial rejuvenation. Both treatments can be done on the same day; Ultherapy first followed by dermal fillers. For patients who may be concerned with the mild swelling which occasionally occurs near the jowls after an Ultherapy treatment, dermal fillers can be added one week after treatment.  

 

5. How does the Ellansé dermal filler compare with other types of filler for lifting and volume loss?

Ellansé is a hybrid biostimulating filler consisting of a short-lasting filler component called carboxymethyl cellulose (CMC) and a long-lasting biostimulator called polycaprolactone (PCL).

When Ellansé is injected into the skin, the CMC provides an immediate lift to volumise deficient areas. PCL triggers collagen production in the skin while CMC is rapidly reabsorbed. Biostimulation leads to a long-lasting rejuvenation of between one to four years depending on the preparation used. At this point in time, Singapore's Health Sciences Authority (HSA) has only approved the use of the S and M version, which lasts for one and two years respectively. 

In my experience, Ellansé gives a very good lift over the cheeks, chin, nose, temples, and forehead. Compared to hyaluronic acid fillers, it has less water retention and compared to pure biostimulators like Sculptra, it has more rapid results.

Ultimately, the decision to use any dermal filler depends on the treatment location, desired results, as well as the patient’s preferences, and the experience of the doctor in using the product.
